This is $15.00 for 15 minutes of work. 

I will email the information for up to 5 medications. I research and see if there is issues with combining the other medications they are on.

This is $30.00 for 30 minutes of work. 

I will email the information for up to 6-10 medications. I research and see if there is issues with combining the other medications they are on.

This is $45.00 for 45 minutes of work. 

I will email the information for up to 15-20 medications. I research and see if there is issues with combining the other medications they are on.

This is $60.00 for 60 minutes of work. 

I will email the information for up to 20-25 medications. I research and see if there is issues with combining the other medications they are on.

It is $60.00 for 60 minutes of work. 

I will make a visit to the persons home and assess them for Home Health or Hospice. 

If he/she is appropriate I will contact the Dr and the families choice of Home Health or Hospice.

*If there is more than one person then add as many people you need to be evaluated to the cart.

Going to the Dr's office with a Client so they get the most out of the visit.  I will visit with the Client to see what the appointment is for and see what the questions they have for the Dr are.  Then I will meet them at their Dr's office. 

Dr's are usually in a rush and you can't get their attention for very long.  I can be very assertive and get his/her attention to get your questions answered.  I have worked with all kinds of Dr's in my career.

After the appointment I will write up the appointment and email it to the Client.

I will update my chart on the Client so if there are new medications, labs, or anything else I will have it for the next visit if they want me to go or if they have issues with the new medications we will have it all in one area and we can remind the Dr what medications the patient is on.  This saves money for the Client and pain if the signs and symptoms are from the new medication then it can be changed to something else per the clients direction.

This is $30.00 for 30 minutes of work.
